Daily Diabetes Control Detail Form

The largest part of the book details the specific things we did on a daily basis in our struggle with the disease. At first, there was no form, just me making notes on scratch paper. Afterwards, I had to pull these notes together into some semblance of order. The end result was the Daily Diabetes Detail form that we used to present the data. I have put a blank form up on my website for anyone to use, free of charge.

Keeping a record is very important. It tells uou where you are, and how you got there. It gave us a sense of how we were progressing. Keeping a journal is a great way to maintain a consistent application of your plan. Showing it to friends is an excellent motivator to stick with it. Showing it to your doctor is an excellent way to communicate with him. It gives you a sense of accountability, and makes you more responsible. It will highlight your accomplishments and pinpoint your mistakes. Periodically reviewing your journal will show patterns as they develop.
